Medication Therapy Management

Medication Therapy Management (MTM) is a service whereby our clinical pharmacist will evaluate your prescription medications, over-the-counter medications, and any supplements you might be taking in an effort to create a comprehensive inventory of these substances, and to identify and/or resolve drug therapy problems such as duplications, under- or over dosing, drug interactions, or other types of drug therapy related issues. Our clinical pharmacist may discover medications that need to be added or stopped, and will include formal communication of such findings to your prescribing provider. Additionally, MTM involves routine follow-up monitoring to determine if symptoms are resolving, if the patient is experiencing side effects, and to assess adherence to the drug therapy.

To begin, our clinical pharmacist with conduct an assessment to determine patient adherence to medication recommendations by your prescribing provider, then, they will provide education to you about monitoring reactions to new and changed drug therapies. In sum, the overall goal of MTM is to enhance appropriate medication use, improve medication adherence, increase detection of adverse drug events, improve collaboration between a prescribing provider and the clinical pharmacist, and ultimately, to improve patient outcomes.
